• Today, 60% of all Americans suffer from at least 1 chronic disease.   40%  of adults have 2 or more chronic conditions.

• Cost - The rising rate of  unhealthy Americans continues to grow.  Those with chronic disease are significantly more expensive to provide medical care for compared to healthier people without chronic conditions.   

The solution has proven to be PHM.    PHM helps prevent chronic diseases, reduces hospital readmissions, and improve patient outcomes. The benefits of our participants include a reduced risk of chronic diseases, improved health outcomes, and increased patient satisfaction.  

Lowering the cost of care reduces premiums which among other things adds to the bottom line.

As we add particiapants, with the help fellow business' in lowering chronic disease,  M1M donates  $5, evenly divided for every enrolled participant in our program at the end of each year to the following national charities:
